PAYMENT

1. How can I make my payments?
You can make your payments via cash, credit card (including via Payment Advise whereby an instruction is given to deduct your dues on a monthly basis by using a Recurring Charges Form or call to 603-5639 9993 to arrange a one-off payment), cheques, bank-in to the SHLSB account or transfers through Maybank2u.com.

2. Where can I remit my payments?
You can remit your payments to the following:-

a. Head Office at Menara Sunway
b. Branches of Sunway Healthy Lifestyle (SHL)
c. *SHL Maybank account number 5123-1651-2659


3. To whom should I issue my cheque/credit card payee?
Cheques or credit card payments should be made payable to Sunway Healthy Lifestyle Sdn. Bhd.

4. When will my payments be due and how will I know when it is due?
Your monthly instalments (if applicable) will be due by the 1st or 16th of the month (depending on the date of purchase) and your Re-enrolment Fees will be due on the first day of the next Anniversary Year.

You will be sent a reminder letter on your dues and other relevant charges.

5. How do I go about bank-in payments?
A) Cash & Cheque - must be banked in over the counter, and fill up the
  i) Keterangan Urusniaga (Transaction Description Field) , and
  ii) No. Rujukan (Reference No.)

B) Cheque - Please slot in the DROP CHEQUE BOX or the SCAN CHEQUE MACHINE, provided that the member fill up the following:
  i) Keterangan Urusniaga (Transaction Description Field) , and
  ii) No. Rujukan (Reference No.)

*Please fax the bank-in slip (confirmation slip) to SHL at 603-5639 9555.

6. Can I pay online?
Yes you can.
a) For CIMB account holder, login to your CIMB Clicks account and please refer to option 1 for further instruction.

b) For other banks account holder (Only for Maybank, Hong Leong, Bank Islam, RHB & Public Bank), please visit www.cimbclicks.com.my and refer to option 2 for further instruction.
