1. APPOINTMENT, RESERVATION & CANCELLATION PROCEDURES
Medical Check-up

To make an appointment for health screenings, please call Sunway Healthy Lifestyle (SHL) Member Services Personnel at +603- 5639 9993.
Appointments must be made at least fourteen (14) days before the intended medical check-up date.
Appointments for health screenings may be done up to six (6) months in advance.
Confirmation of appointments is always subject to availability.
Any cancellation or change of appointment has to be informed at least seven (7) days prior to the appointment date. This Subscriber's health screening benefit for that particular Subscription Period will be forfeited otherwise.
No Subscriber's benefit(s) may be carried forward to the following Subscription Period.
The Subscriber's health screening will be on a first-come-first-serve basis on the confirmed appointment date which starts at 9.00am. Please note that it would not be possible for the entire screening programme to be completed in one (1) day if patients arrive later than 9.30 a.m.
Please note that the investigations will take up to 3-4 hours in total. Appointment with the doctor to discuss investigation results will take place in the afternoon of the same day.
The Subscriber's health screening benefit is only available upon payment of 40% of the Subscription Fee and provided all other payment dues must be current.

Hotel Room Reservations

The SHL Hotline (603. 5639 9993) is also the contact number to make reservations for the Resort Suites at Sunway Lagoon Resort Hotel and other hotels which SHL deem fit to provide for its Subscribers from time to time at SHL's sole discretion.
Reservations are subject to availability and are to be made at least fourteen (14) days prior to the intended check-in date.
The Subscriber needs to notify SHL of any cancellation or change of reservation details at least seven (7) days prior to the check-in date. This Subscriber's hotel stay benefit for that particular Subscription Period will be forfeited if not adhered to.
The hotel stay-benefit is only available upon payment of 40% of your Subscription Fee and all other payment dues must be current.

2. MEDICAL CHECK-UP PREPARATION
General Advice
Please do not wear jewellery or other valuables as it may interfere with the medical check-up. Please note that any lost belongings will be at the sole responsibility of the Subscriber and not SHL nor the Sunway Wellness & Aesthetics Centre.
For those wearing contact lenses, it is suggested that spectacles be used. Otherwise, please bring along your contact lenses container as the lenses need to be removed during the eye examination.
Fasting is required from midnight before the day of the health screening. No food or beverages are allowed during the fast, with the exception of plain water
The purpose of fasting is to achieve accurate results for your blood tests for glucose, cholesterol and triglycerides
Please do not take any medication on the day of your appointment.


Ladies

It is advisable to book your appointment around Day 10-14 of your menstrual cycle. This is about 5 to 7 days after your last day of menstruation
If you are menstruating on the day of your appointment, please re-schedule your appointment. This is because the urine and cervical (PAP) smear test cannot be performed if you are menstruating
Please note that only ladies who are sexually active are advised to have cervical (PAP) smears.
If you are pregnant or suspect that you may be pregnant, radiological investigations e.g. Chest X-Rays, Fluoroscopy X-Rays, CT Scans or Mammograms are not recommended
For ladies undergoing Mammogram testing, please do not use deodorant or talcum powder on the day of the Mammogram test.

Stress ECG (treadmill) tests

This test takes about 45 minutes to 1 hour.
Please bring along your sports attire and running shoes for this test.
Avoid having a heavy meal before this test.
If you are on heart medication, please inform your doctor before starting the treadmill test.
 
AT THE CENTRE
Please approach the registration counter upon arrival for your health screen.
Please produce your Appointment Confirmation Slip & Subscription card. Please highlight to the Customer Relations Officer if you are pregnant, fitted with a pace maker, have allergies, wearing contact lenses or are having any health problems etc.
Switch off your hand phone as it may interfere with the medical equipment.
Kindly remove all your personal belongings, such as watches, jewellery, etc as it may interfere with the equipment used.
When you are ready, the Customer Relations Officer will guide you to your first medical check-up procedure.
After each check-up procedure, the Customer Relations Officer will guide you to the next procedure. If you are unsure or have any queries, do check with the Customer Relations Officers who will provide you with the guidance you require.


Conclusion

After all the procedures are completed, please check with the Customer Relations Officer regarding the time you are supposed to return in the afternoon to see the doctor for a consultation.
You can enjoy a complimentary snack and beverage at the food & beverage outlet at the hospital, while waiting for the results.
You will be called into the Consultation Room for a discussion and a doctor will explain your results.
The Official Medical Report will be prepared and sent to you by post within a week of your health screening.
