Appointments must
be made at least fourteen (14) days before the intended medical check-up
date. |

Appointments
for health screenings may be done up to six (6) months in advance. |
 |
Confirmation
of appointments is always subject to availability. |
 |
Any cancellation
or change of appointment has to be informed at least seven (7) days
prior to the appointment date. This Subscriber's health screening
benefit for that particular Subscription Period will be forfeited
otherwise. |

No Subscriber's
benefit(s) may be carried forward to the following Subscription Period.

The Subscriber's
health screening will be on a first-come-first-serve basis on the
confirmed appointment date which starts at 9.00am. Please note that
it would not be possible for the entire screening programme to be
completed in one (1) day if patients arrive later than 9.30 a.m. |

Please note that the
investigations will take up to 3-4 hours in total. Appointment with
the doctor to discuss investigation results will take place in the
afternoon of the same day. |
 |
The Subscriber's health
screening benefit is only available upon payment of 40% of the Subscription
Fee and provided all other payment dues must be current. |
